"about": "

About this record
\n
This page compiles the federal actions, standards, court decisions, and industry milestones that shaped air medical transport in the United States. It describes what each instrument required or found and what became of it. It does not argue for or against any position.

\n\n
Who controls what
\n
The FAA regulates aviation safety. Helicopter air ambulance operators hold Part 119 certificates and fly under 14 CFR Part 135. The FAA acts through rules published in the Federal Register and codified in the CFR, and through instruments that are not rules. These include operations specifications issued to individual operators, advisory circulars (guidance), notices and handbook bulletins to inspectors, safety alerts, and technical standard orders for equipment.
\n
The NTSB investigates accidents and issues safety recommendations. It has no regulatory authority. It classifies each recommendation by the recipient's response, from Open, Acceptable Response through Closed, Unacceptable Action.
\n
Congress can direct rulemaking and set deadlines. It did so for air ambulances in 2012, 2018, and 2020, and amended the 2012 provisions in 2024.
\n
The Department of Transportation holds economic authority over air carriers. Under the Airline Deregulation Act of 1978, states may not regulate an air carrier's prices, routes, or services. Courts have applied this to air ambulance operators.
\n
The states regulate medical care, including crew qualifications, medical equipment, protocols, and medical dispatch. The line between medical regulation and preempted aviation or economic regulation has been litigated repeatedly.
\n
CMS sets Medicare payment. CAMTS is a voluntary accreditor whose standards some states adopt as a licensure condition. Industry associations (AAMS, VAI, AMOA, NEMSPA, AMPA, ASTNA, IAFP, and others) publish voluntary initiatives and testify in rulemaking.
\n\n
How to read an entry
\n
\n
Primary sourceTraced to the governing document, such as the Federal Register, U.S. Code, eCFR, an NTSB report or letter, or a GAO report.
\n
Secondary sourceDrawn from a reliable secondary account. The primary document has not yet been confirmed.
\n
Verify before citingRests on a single secondary source or contains a detail not yet confirmed. Shown so the gap is visible.
\n
\n
NTSB recommendation statuses were retrieved from the NTSB CAROL database on October 26, 2026. CAROL is the authority for current status.
\n
Accident and fleet counts differ by source because definitions differ (dedicated versus dual-purpose aircraft, inclusion of public-use operators, fixed-wing). Each figure is shown with its source.
\n\n
Scope
\n
Helicopter and fixed-wing air ambulance operations in the United States. Federal statutes, FAA rules and guidance, NTSB studies, reports, and recommendations, GAO and Inspector General audits, preemption decisions, accreditation, and major industry events. Individual accidents appear only when they produced an investigation report or recommendations that shaped later policy.
\n\n
Method
\n
The record was built from the NTSB special investigation (2006), the NTSB recommendation letters of 2009 and 2013, the 2010 proposed rule and 2014 final rule, the current eCFR, 49 U.S.C. 44730, 44731, and 44737 with their amendment history, FAA reports to Congress, GAO reports and testimony, the DOT Inspector General's 2015 audit, the 2015 DOT guidelines, the 2016 NASEMSO model rules, and NTSB accident reports. Secondary accounts were used to locate primary material. The active issues section was researched from bill texts, rulemaking dockets, agency pages, and the published positions of the organizations named.
\n\n
Maintenance
\n
Active issues are reviewed monthly. The rest of the record is reviewed quarterly and whenever a new rule, NTSB report, or statute is issued. Corrections and additions are welcome. Please include the primary source.
\n\n
Limits
\n
This is a research reference, not legal or regulatory advice. Operators should rely on the current text of the regulations, their operations specifications, and their principal inspectors.
\n"
